Usually inner cotton does not come off when you uncap a marker. However if the residue of ink on the body is firmly stuck to the cap like superglue, it may happen.
To put everything back in place, remove the residue of ink attached to the inside of cap and around the nib first, then press the cap and inner cotton into the body of marker together.
Also if the marker has cracks on its body, it will cause the same problem. Ink in a cracked body will be volatilized fast and there’s no method for restoring, unfortunately. Only thing you can do is purchase a new marker.
